Thousands of Indian students are sold a dream of "affordable MBBS abroad" every year. The reality — backed by official NMC and NBEMS data, government gazette notifications, and NMC Act 2019 — is far more challenging and financially dangerous than agents will ever admit.
Fact #1 — 70–80% of Foreign MBBS Graduates FAIL the Indian Licensing Exam
Every Indian citizen who completes MBBS abroad must pass the FMGE — Foreign Medical Graduates Examination (conducted by NBEMS, natboard.edu.in) before they can get registered to practice medicine in India. This is mandatory under the NMC Foreign Medical Graduate Licentiate (FMGL) Regulations, 2021. The pass rate across sessions has consistently been between 10–30%. In June 2023, only 10.20% passed. In July 2024, of 35,819 candidates, 27,297 FAILED (79.8%). There is no limit on the number of attempts — meaning graduates can spend years and lakhs attempting to clear an exam before they can even begin practising.

NMC de-recognised BS route (2021) — verify before paying
The National Medical Commission (nmc.org.in) has officially stated it does not endorse any list of foreign medical institutions. The erstwhile MCI list is outdated and carries no legal weight. Students are solely responsible for verifying recognition, fees, and course structure directly from the foreign university before paying any amount.
Double Internship Trap
Mandatory 2nd Internship in India — 1 More Year Lost
Under the NMC Compulsory Rotating Medical Internship (CRMI) Regulations 2021, foreign MBBS graduates must complete a mandatory 1-year internship in India even after completing one abroad. This is in addition to passing FMGE/NExT. Agents almost never mention this — it adds one full year and significant cost after your return.
No Clinical Exposure
Students Often Cannot Touch Patients
In many foreign programmes — especially in Russia, Kazakhstan, Kyrgyzstan and China — Indian students are not permitted to examine or treat patients due to language and licensing restrictions. MBBS in India includes a mandatory integrated 1-year internship with direct hands-on clinical training. This practical gap is the primary reason for the catastrophic FMGE/NExT failure rates.
Philippines BS Route — Banned
NMC De-recognised Philippines BS Course (2021)
Under FMGL Regulations 2021 (NMC), the pre-medical BS course route in the Philippines was officially de-recognised from 18 November 2021. Students who enrolled via the BS route after this date are ineligible for FMGE or NExT and cannot legally practice in India. Many consultants are still selling Philippines seats without disclosing this.

Tougher NExT Exam Replacing FMGE
India is replacing FMGE with the National Exit Test (NExT) under Section 15 of the NMC Act, 2019. NExT will be a single, unified exit exam for all MBBS graduates — Indian and foreign. It is expected to be significantly harder than FMGE, further reducing the already dismal chances for foreign graduates who lacked proper clinical training during their MBBS.

Agents Quote ₹20L — Reality Has Hidden Costs
Agents advertise "₹18–25L total." The real direct cost — tuition, hostel, food, flights, visa renewals, and FMGE/NExT coaching after return — typically reaches ₹35–45 lakhs. Add delayed careers from slow exam cycles, unpaid mandatory internships, and years of lost income — and the financial drain becomes enormous. A private MBBS in UP or WB costs more upfront (₹55–80L over 5.5 years), but gives superior clinical training. Either way, both Indian and foreign graduates now face the NExT exam (NMC Act, 2019) before practicing. If budget is a concern, also consider BDS (from ₹3.2L/yr) or BAMS (from ₹50,000/yr) — strong careers with far lower cost and zero licensing hurdles.
Georgia / Armenia PG Lies
False PG-in-UK / Royal College Promises
Consultants pushing Georgia, Armenia, and Azerbaijan seats frequently tell parents: "Our college has a tie-up with a UK Royal College" or "You can do PG in Germany after MBBS here." These claims are almost entirely false. No foreign MBBS degree automatically qualifies for UK PLAB, USMLE or EU licensing — each requires separate qualifying exams, years of additional training, and sponsorship. These promises have no legal basis.
South America Trap
USMLE-via-Peru / Caribbean — A Dangerous Scam
A growing number of agents in Northeast India are promoting MBBS seats in Peru, Belize, Caribbean islands, and similar destinations with promises of "easy USMLE pathway" or "US residency route." These institutions have FMGE pass rates below 5%. Most are not NMC-recognised. Students are left with a degree they cannot use in India or anywhere else — after spending years and crores. Always verify on nmc.org.in before paying a single rupee.
